We were 3 carers and my disabled daughter and I registered 1 extra medical case and a manual wheelchair, so 5 cases in total plus the wheelchair and had lots of room in our people carrier transfer.
I always pay for the transfer in Euros on my Starling card as they give me the best exchange rate.
